Summary of Employee Benefits
Full-Time employees of the Indian Community School of Milwaukee, Inc. (the “School”) are eligible for certain employee benefits. Employer paid benefits for employees and any eligible family members begin on the first day of the month following 30 days of employment with the School unless otherwise noted.
Group Medical Insurance is provided through UnitedHealthcare. The UnitedHealthcare Choice Plus Plan Balanced 100 - 20/1000/100% PlanW2F gives eligible employees the freedom to see any Physician or other health care professional in the Network, including specialists, without a referral. In-Network benefits are paid at the highest level available, less a 20 dollar co-payment for office visits. Care sought outside of the UnitedHealthcare Network means a higher deductible and co-payment. If an election is made not to apply for coverage when eligible or enrollment is not completed within 31 days of eligibility, the employee will be subject to an evidence of insurability process, and Plan pre-existing condition clauses. The UnitedHealthcare Choice Plus Plan Balanced 100 - 20/1000/100% PlanW2F open enrollment period is January 1 through January 15. Employees pay their health insurance premium contribution with pre-tax dollars through a bi-weekly payroll deduction. Annual employee premium increases are calculated on January 1.
Group Dental Insurance is provided through MetLife. This is a Preferred Provider Option (PPO) Plan, and when care is arranged with a Preferred (network) Provider, benefits are paid at the highest level available. When care is arranged out-of-network with a Non-Preferred Provider, benefits are paid at a reduced level. If an election is made not to apply for coverage when eligible, the employee may apply for coverage during the next open enrollment period and will be subject to a two year waiting period for orthodontics and prosthodontics. The MetLife PPO Plan open enrollment period is July 1.
Group Vision Insurance is provided through National Vision Administrators, Inc., and when care is arranged with a Participating Provider, benefits are paid at the highest level available. Benefits are paid at a reduced level when care is arranged through a Non-Participating Provider. If an election is made not to apply for coverage when eligible, or enrollment is not completed within 31 days of eligibility, the employee may apply for coverage during the next open enrollment period. The National Vision Administrators, Inc. plan open enrollment period is November 1.
Group Term Life and Accidental Death and Dismemberment Insurance benefits up to 10,000 dollars, are provided through Reliance Standard Life Insurance Company.
Group Short Term Disability Insurance is provided through Northwestern Mutual Life Insurance Company and replaces a portion of the income of employees who are too sick or hurt to work on a full-time basis. Employees that meet plan eligibility requirements, will receive benefits, if they become disabled while insured. The short term disability benefit payment period will normally begin on the eighth day for either injury or illness and will continue for 26 weeks.
Group Long Term Disability Insurance is provided through Northwestern Mutual Life Insurance Company and provides monthly income to employees who are unable to work or have a reduced income due to injury or illness and may continue to Social Security Normal Retirement Age.
Paid Time-Off will be granted to employees beginning July 1, of each year. School Year employees will be granted 8 days of paid time off and Year Round employees will be granted 10.5 days of paid time off during each Fiscal Year (July 1 through June 30). Any unused paid time off prior to the last day of the Fiscal Year will be paid to the employee. Employees hired after July 1 will be granted Paid Time-Off during the first fiscal year of employment that has been prorated based upon their hire date.
403 (b) Tax Deferred Annuity Plan with the Principal Group. All employees except those who normally work less than 20 hours per week are eligible to participate in the plan. Employees may make elective deferrals immediately after completing a salary reduction agreement. Employees are eligible to receive employer matching contributions of 50 cents per 1 dollar contributed up to 10 percent of their annual gross salary, and employer discretionary contributions, currently 5 percent of their annual gross salary, after having completed 1 year of service and attaining the age of 21. Employer matching participation entry dates are July 1, or January 1, following fulfillment of plan requirements.
Employee Assistance Program provided by Southeastern Wisconsin Medical and Social Services, a member of Covenant Healthcare System, Inc. The Employee Assistance Program provides employees and their families with a professional, confidential program to assist them in resolving personal concerns. Employee eligibility begins on the first day of employment.
Flexible Benefit Plan provided by ProcessWorks, Inc. The Flexible Benefit Plan is a type of Section 125 Plan which allows employees to pay for certain expenses with pre-tax dollars. Qualifying expenses include deductibles, coinsurance, co-pays, prescription drugs, eyeglasses, day care services, and privately owned insurance policies. Employee eligibility begins on the first day of employment.
