“Families In Need” Program

Applications for the “Families In Need Program” will be accepted starting October 1, 2008. Families wishing to fill out an application for Thanksgiving and Christmas vouchers my do so at the following times:

Monday’s 10:00am-1:00pm
Wednesday’s 2:00pm-4:00pm
Thursday’s 4:00pm-6:00pm *No applications taken on October 30, 2008

These are walk-in appointments, no appointment is needed. If you are unable to
apply during these times you may contact the FRC at 525-6117 to make other
arrangements.

NO APPLICATIONS WILL BE TAKEN WITHOUT AN APPOINTMENT OR OUTSIDE
ABOVE LISTED TIMES.

Please note the following deadlines for each program are as follows:
Thanksgiving Vouchers: October 1, 2008-November 7, 2008
Christmas Vouchers: October 1, 2008-December 5, 2008

In order to fill out an application you must have:

Tribal ID and/or Tribal number
Proof of income for ALL adult members in the household.
Applicants must interview with FRC staff.

*Please note applications will not be accepted without the above information.
Applications will be considered based on:

-Must have a child who attends Indian Community School or Family Resource Center
programming.
-Income level.
-Information obtained in interview with FRC staff.
-Past services received.
-All school records/contact information must be up to date.
